Care Management Assistant needed in Bromley

This is a temporary role and the rate is £18.03ph PAYE

The successful candidate will deliver high‑quality Care Act assessments and planned reviews, ensuring that each adult’s needs, outcomes and support arrangements are accurately understood, proportionately evaluated and clearly reflected within person‑centred care and support plans. They will work confidently across a range of settings — including care homes, Shared Lives placements, Extra Care Housing and domiciliary care arrangements — undertaking visits both within and outside the borough to review individuals in their current placements.

They will collaborate effectively with care providers, housing teams, health professionals, families and advocates to maintain continuity of care, identify and manage risks, and implement appropriate safeguarding measures. The successful candidate will demonstrate strong professional judgement, completing Mental Capacity Assessments and contributing to Best Interest decisions where required, while maintaining timely, defensible and compliant case recording in line with statutory and organisational expectations.
